SEMPSL Player Registration Form

Every new player must fill out an "Amateur Player Registration Form". Team managers should fill out the "Team Information" area and players should fill out the "Player's Information" area.
Also, team managers need to complete the "Team Affiliation Form" and submit it with all the Individual Registration (AP Reg Form and Players Cards).

It is very important that everything is filled out correctly to ensure that the player's registration is not rejected by the state. Below is a list of guidelines to help you fill this form out correctly.

PLAYER INFORMATION
LINE 1:	Enter the player's last name, first name and middle initial (if one exists).
	Indicate the gender of the player.
	Lastly, enter all digits of the player registration number (either from a "Player Card" or a "New Player Reg. Number" for "New Players")
LINE 2:	Player's address: street address, city and zip code. Note: player's should use their local address. Phone Number is Optional
LINE 3:	Player's date of birth and country of birth. Indicate which state the player has played soccer other than MI (if applicable). Do the same for "country."
LINE 4:	If the Player has played abroad, then fill out this portion. Leave blank if not.
TEAM INFORMATION
LINE 5:	State code (26) and State Association (Michigan Soccer Association) - this should already be on the form. Enter "SEMPSL" as the "League"
LINE 6:	Enter "Teams Full Name." Example: "AASC Ajax," not just "Ajax." So include Club or Organization name. Enter Division (Either 1 or 2)
TEAM MANAGER INFORMATION (this is for the "Teams Manager" to fill out.
LINE 7:	Enter Team Manager's Last and First Name. It would be good to enter the Email Address as well.
LINE 8:	Manager's name, street address, city and zip code. Also enter the team managers "Phone Number"
RELEASE AND DISCLAIMER
LINE 9:	The player must sign and date the form.
LINE 10:	The manager must sign and date the form.
LINE 11:	LEAVE BLANK.
